We are excited that you have chosen to come see Dr. McGrew! There are a few important things that need to be done before your first appointment. Please read the following very carefully and complete each step.
1.) Contact your primary caregiver and/or your insurance company to find out if you need a referral to see Dr. McGrew. If you do, the referral needs to be sent to Dr. McGrew before your first appointment.
2.) Contact our billing office (Sound Management; 206-592-5000) and check to see if we accept your insurance. This is an imperative step! Depending if we do or not will greatly impact the amount you are responsible for.
3.) Gather any information you feel is relevant to your child's issues. This may include clinic notes from other offices, previous test results, previous assessments, letters/notes from teachers, school IEP, a personal account of events/concerns, etc.
NOTE: When requesting information to be sent to our office from clinics, hospitals, schools, and other professionals, a "Release of Information" form will need to be signed giving each party permission to communicate. Also, it is best to have other parties directly fax/mail us information about your child ahead of time instead of hand carrying the items yourself. It will quicken the intake process and make your time at our office as valuable as possible.
4.) Download the "Patient Intake Form" and complete. To maximize your time at our office, Mail/Fax/Email the intake form before you come in.
